You can choose which surname you wish to use and how you want the municipality to address you. Official documents usually only show your birth name. However, when applying for a travel document and / or driver's license, you can indicate that the name of your spouse should also be stated.
Which surnames can I choose from?
You can choose from:
- your own surname;
- your own surname followed by that of your partner;
- your partner's surname followed by your own surname;
- your partner's surname.
You can reverse your decision later if you change your mind.
Official documents always use your own name.
How can I notify the municipality of which name I wish to use?
Go to Change surname to be used (only in Dutch). Log in using DigiD and complete the form in the screen. You will receive an e-mail confirming your request and an e-mail confirming the changes.

Click on Change surname to be used (only in Dutch). Enter the details in the screen and then print the form. Sign the form and send it, enclosing a copy of your valid passport or identity card to:

You will receive written confirmation of the surname you have chosen to use, within 7 working days.
- At the counter
You can also notify us of your choice of surname at the appropriate counter at Residents' Plaza (in the Stadskantoor). You can make an appointment online or by phone (14 040). Please bring a valid identity document with you.
