Can I apply for the certificate in a different way?
I need the certificate for a pension payment
Besides using the online application form with DigiD (only for residents of Eindhoven, and only in Dutch), you can apply in the following two ways:
By post (only for residents of Eindhoven)
Fill in the information on the print form (only in Dutch), print it out and send the signed form, together with a copy of your identity document, to the address on the form. Go to the central government website to see how to safely send a copy of your proof of identity (info in Dutch)
At the counter
Make an appointment and come to the Residents' Plaza. Bring your identity document with you and pay by direct debit card (preferable) or in cash.
If you apply at the counter, the certificate will be issued to you immediately. If you request the certificate online, or by post, we will send it to you by post within 5 working days.
Can I also apply for a certificate for another reason
Yes, you can. Request the Proof of Life certificate (other purposes) online with DigiD (only for residents of Eindhoven and only in Dutch). Once you have completed the form, you can pay immediately online with iDeal. The certificate will be sent to your home address within 5 working days.
Can I reply for international version?
That's possible. The international version is an Attestation de vita. You may need an Attestation de vita for a foreign organisation, for example. This certificate is valid both in The Netherlands and abroad. Make an appointment online to apply for an attestation de vita. Come to the counter and bring a valid identity document with you. You can pay in cash or by direct debit card and you will receive the certificate there and then.
Wat is het verschil tussen Bewijs van in leven zijn en Attestatie de vita?
Op een Attestatie de vita of een buitenlandse formulier Bewijs van in leven zijn wordt door de Ambtenaar verklaard dat je in leven bent op het moment dat het bewijs wordt afgegeven en is meestal bestemd voor de buitenlandse instanties.
Een Bewijs van in leven zijn is een uittreksel uit de basisregistratie personen. Op dit uittreksel, anders dan bij een attestatie de vita, wordt niet verklaard dat je in leven bent.
Can I apply for a certificate if I am not registered in The Netherlands?
That's possible. If you are in possession of Dutch nationality but are not registered anywhere in the Netherlands, you can visit our counter with a valid Dutch passport or identity card. Make an appointment online first. You pay in cash or by direct debit card and you will receive the certificate there and then.
Can someone else apply for the certificate on my behalf?
Certainly. You can authorise anyone you choose to apply for a Proof of Life certificate on your behalf. The authorised person comes to the appropriate counter, with the following items:
- An authorisation signed by you,
- A copy of your identity document,
- His or her own identity document.
If you are unable to come to the appropriate counter, because of a severe physical or mental disability, you can authorise someone to apply for an Attestation de Vita. In addition to the above-mentioned documents, the authorised representative also brings with them a written declaration from the physician or care home stating that you are unable to come to the counter in person.
